I see comments that this is not real leather. Which is true? And what does "hand-synehtesized" mean?

It's not plastic, as someone else commented. It's waxed cotton which gives it a leather feel. Good for cheap jewelry, but it doesn't really last as long as real leather.

At the given price, I doubt the metal is anything but plated iron. The product description should specify the make-up of the parts.

Can you fit sea shells on this necklace?

It depends on how you want to hang the shell from this cord? If you drill a hole on the shell and put a jump ring thru the hole that would work.

What are the 5 colors ?

It's been so long since I ordered them, but I do remember that a lot of them were black, and some brown, tan, cream; I think. There weren't bright colors like red or blue. Sorry I can't be 100% sure. I just remember getting them because the price was pretty good for so many.
