I feel like I'm falling forward in my chair.. did I do something wrong?

When I built mine, it rocks a little back, but not forward. You might need to tighten up some of the bolts.

how should I lock the back of chair ?

The handle that makes the chair go up and down...push it in to lock back and out to loosen back of chair.

Is this chair suitable for a carpeted floor?

I have laminate floors but it rolls just fine on the rug.
