Q: Does this letterbox cover attach to upvc door?

It can be attached to a PVC door, but please note this does have to be screwed into place, so please take note of the dimensions of the letterplate and the apperture (opening) to make sure it covers the whole fully.

Q: Is this suitable to be fixed vertically

I fixed it horizontally but I don't see why it couldn't be fixed vertically. The spring will always bring the flap back down.

Q: is there an internal and mid section to complement this?

Not sure what you mean by a mid section but the mid section is filled with the brushes. I've recently had a new (expensive) door and the letterbox is not as good quality as the Stormguard.
