H&P Notebook: Medical History and Physical Notebook, 100 Medical templates and Free Bonus

I'm a nurse and I am stressed out and overworked right now more than any other time in my career. I'm willing to buy anything that will help me stay organized while I'm trying to take care of my patients. This book of H&P templates will make it easier to take thorough notes while I'm with the patient so I can write my shift notes later during my lunch. It beats the folded-up sheets of paper I’ve been using.What I liked:It's large enough that you have space to write in each section but small and flexible enough that you can fit in the pocket of your lab coat or scrubs.It has lists of common abbreviations, lab values, and conversions in it for when my brain is a little tired. And they also give you the tables as a free pdf for your phone or tablet.These would also make a nice little gift for someone in nursing or med school. I actually used to use something similar to this in school but it was loose pages and not bound, so this would have been more convenient.What I didn’t like:Nothing so far. The description was pretty accurate so I knew exactly what I was getting when I ordered it.

This book looked exactly what was pictured and inside did not disappoint. It has areas for the basics like date of admission, name, room, bed number, etc, but also lists the body systems so you cannot miss doing a head to toe assessment and forget one. I also like that it has an area for the labs in a clear way and reminds you which goes where. When I first became a nurse this was hard to memorize at first, but over time it made telling physicians what the labs were that much easier. I highly recommend this book, especially if you're just starting out as a nurse.

The media could not be loaded.  I got this book because I love everything by Medical Creations and it is awesome. Understand it is meant for providers rounding on patients but I can even see this being helpful for other uses. Specifically nurses in home health might use this to keep up with their admissions or patients! The back has a cheat sheet that is also helpful. The soft cover is sleek, the pages are good quality. Great buy. I’m tellling everyone I know about this book.

I really like the compact nature of the book and how it can easily fit into my lab jacket. Easy place to jot down patient notes prior to notating in the EHR.

It’s a tiny book, difficult to fill in near the spine. The pages are not perforated to rip out. There are no guides included to reference, or prompts based on systems.Why bother?

The h+p pages are perfect for Physician Associate students, and the extra guides at the back are so helpful. My only improvement would be additional notes pages throughout
