Q: Is this compatible with Garmin instinct

I not saying you couldn't make it work but what I can tell you is I wouldn't. I had this band on a Casio very briefly because the way it attaches to a watch it sits it very high off of your wrist causing your watch to catch on everything you pass. For the record I owned an Instinct also before upgrading to a fenix. I would stick to bands specifically for your Instinct. It's a great watch btw. My father in law still uses my old Instinct and loves it.

Q: Will this work with 18mm lugs

Large watches fit better but a jeweler might weave the pin thru to make it work.

Q: Does this interfere with heart rate monitor?

Forget John Wick's review. For any heart rate monitor to work, it needs to be in contact with the skin. Your smart watch is gonna sit above the band so the heart rate monitor function won't work.
