Can i use the spout without the straw?

You can. My kid likes to drink it with no straw and it makes no sound , although she does not suck super hard yet

Is this recommended to keep my baby's milk cold?

It is well insulated, but if you are putting it in a lunch box or something then I would also recommend an ice pack.

What size is the straw used? I would like to get replacement stress for this kids water bottle.

It's the exact same straw the 12 oz Thermos Funtainers use. They are 5 inches or 12.7 cm. We use them as well so it's nice to be able to swap them out if needed.

Is this bottle too cold to hold?

No. My kindergartner used it all year at school without any issues.
