Does this have a plain smooth cotton side that isnt fluffy?

There is not much difference in the sides they are both smooth woven and the "fluffy" side is not very fluffy at all. Certainly not fluffy enough for the fabric to stick to a wound.

What is the width and length of the roll?

As I've already used some of it I can't tell you the length of the roll, however, the width appears to be approx 11".
