Is the shower spray mode a fine gentle mist? or a strong fast blast?

It varies of course on how good your water pressure is and how wide you open the faucet. At my house with the faucet fully open it has a good spray, not overly powerful as to sting your hands, but a good spray to work with. By turning down the pressure you can achieve a lighter spray but not a mist. We are really pleased with it and it has a great rotation for reaching the corners of the sink. My originally didn't think the degree of angle was that good but I found that it will go to a greater angle than what at first didn't feel comfortable. It is a great add on and well worth the money IMO.

Does it fit an 18" portable dishwasher

With the purchase of two extra parts which was about 15 dollars total , I was able to hook this up to my pull out kitchen sink faucet which is larger than the normal faucet size to the Quick connect to the portable countertop dishwasher i have. (the Danby 6 Place Setting Countertop Dishwasher - Stainless Steel) but you probably can't do it without adding a coupling or two to adjust to the sizes.

Why so many options? How do i know what to get? I know i have a male faucet soo........which one do i buy?

You need female thread aerator to fit for your male faucet. Measure diameter of faucet before order. If Male 55/64"-27, it will fit.
