Q: how often shoudl the filter be changed?

Almost never. When you lose it or it breaks.

Q: Do you have bigger diameter? and does it work for hot brewing?

Since this is made for a fairly specific collection of Yama brewers, this is the only diameter. It can be used in hot brewing as a supplement to a permanent mesh-type filter, since it's meant to catch fine sediment that metal filters might let escape. Check out the Yama Silverton model as an example.
