Does this fan work on the ps4 slim

no, only works for PS4 CUH-1200 CUH-1200AB01 CUH-1200AB02 CUH-1215A CUH-1215B. This cooling fan DOES NOT work for PS4 CUH-10XXA and 11XXA/B, PS3, PS4 Pro and Slim.

Does this fan come with the proper connector? Ive read a few people had to splice their old connector to this fan. Is this the case?

yes, connectors: 3-Pin 3-Wire, as the picture shows.

why is the thermal paste included ?

The removal of the heat sink is required to replace the fan. And it is part of doing the job correctly to clean the old thermal paste off the CPU and reapply. I would personally invest in a higher quality brand at this time if it were my machine. But I have used the included paste and it is adequate for the job.
