After much use the sheath is falling apart. Is a replacement sheath offered?

I've not had this problem but you can possible find a shoe/boot repair shop. They could repair the sheath.

Does the sheath have any retention to prevent blade from falling out?

The sheath does not have any features intended to secure the blade. The sheath is fairly tight, however it is not of very high quality and will loosen up over time, allowing the blade to fall out when running or anything really. Bought this for carry, but it really should just be a decorative item to display on your desk.
