It should have been attached to the cloth to begin with, so I would say yours may have been defective. The cloth is to keep the oils from the fingers from touching the rosin/hairs to my knowledge.

Try using less when you apply it. It really is designed for use on a cello bow. I have never tried the light version. You might try it.

Q: How many times do brand new strings have to be rubbed on this to get sound from a brand new violin?

You don't rub strings with rosin. You put the rosin on your bow. Them play your strings.
