What is the country of origin? Where made?

Like everything else in your whole world, they're made in China. Nearly nothing is ever made anywhere else any more. Even things that say they're made elsewhere are nearly always constructed of component parts made in China. You just have to get used to that.

What's the temperature rating for these terminal connectors?

The temperature is not listed so I wouldn't use this in extreme temperatures. They say its a vinyl boot (PVC) so -20-80c would probably be ok but more and id start getting concerned.

does this spade have a solid tube inside, for a permanent connection ?

Yes it does and it can be crimped and soldered or both if you want.

What is the voltage rating on these terminal connectors?

If I remember right it is 300 volts for indoor use and appliance use and 600 volts for out door sign use.
