Q: How do you install these?

There is a dimple on the bottom of your mag. Push this in with a hex wrench which will fit the hole or similar size screwdriver. The follower is behind this with the spring behind the follower. Gently slide the face plate off while sliding your finger on to the follower. Be careful as spring is under high pressure. Then just slide pinky extension in its place. Any gun dealer can do this for you too in about 30 seconds

Q: I can hold the spring in but the extension doesn't seem to fit the grooves of the slide?

I thought so at first, but with patience they’ll line up properly

Q: will this fit a P365 magazine?

Package clearly states Grip Extender for Sig Sub Compact P320/250, 9mm/357/40 and they fit on my P250 9mm subcompact 12rnd mags. Package has two mag extenders. Test fit them without the spring so you can see how it works before installation.
