Can you replace the biro with your own pens?

It’s designed specifically for the BIC Cristal pen. The hole is the exact same shape. It may fit other pens but, it would be at your own risk.

Does this work for left handers?

Yes it will do. I use mine all the time at work. It's really chunky and didn't take me long to adjust writing with it

What way do you hold it? Fish eye to the side or facing up?

The tail goes at the nib end. I didn't find it comfortable to hold but found a "fat", round and smooth pen much better such as Lamy or Berol.
