I would like to apply velcro to the boom mat in order to start collecting patches. Would I be able to safely apply an adhesive and add the velcro?

I don’t think that would be advisable. The boom mat has a very thin top membrane and I think it would tear.

Do velcro backed patches stick to the headliners?

No. The headliner has pleather on the top. I'd suggest perhaps using some 3M backed Hook and Loop and sticking it to where you'd like your patches.
