This pillow does not lie flat at the bottom like in the picture it seems to be at an angle ???

Yes...there is a small angle - approx. 20 degrees at the bottom. A good product for reflux (which is a risk factor for oesophageal cancer). The pills - PPIs- are not safe to use in the long term. My irritating dry cough has completely resolved using this pillow, combined with eating dinner a bit earlier. I'm pleased I got it and use it every night. Its comfortable although feels slightly odd the first night only. The cover is removable and washable. The product would be great rather than good if it came with a spare pillow case

Are there ant pillow cases or spare zip-on covers for this?

I haven't found a pillowcase that fits the wedge. However I bought a travel cot fitted sheet from Mothercare and it fits perfectly. 2 in a pack so have a spare for washing!

Is it comfortable to combine this with a regular pillow? I'm concerned that doing this might cause some neck pain.

Hi - an additional pillow makes it more comfortable, it doesn't make your neck hurt as the pillow rise starts from your lower back not directly under your neck - the pillow is quite long. Hope that helps.
