Does anyone have a pdf of the datasheet that comes in the box? ours was damaged and i'd like to keep a record of it for future reference.

I probably can find one, but it's worthless. I tried to figure it out (no dice) then took it to the local motor shop and they didn't understand it either. Typical chi-com gibberish with no explanation of what it does or how to hook it up. They said bring in the VFD and that thing and we can probably get it working...if it works at all.

Is the supply voltage (15v -30v) put in line with the current wires or are there separate terminals for that? The pictures dont show any?

I use this to simulate a 4-20mA sensor(in my case a 24v current loop) I do not need a separate power supply as the 2 wire current loop is both the power supply and the signal path. I would consider a different device if you need to supply power to something. There are many other devices like this that use a power supply and send out voltage or current.
