This should not be used to contour or support the neck while you sleep. It has pressure points and I imagine waking up from sleeping on this would not be a pleasant experience. It works well to release tense muscles in the neck and shoulders and stretch the neck. It seems like I read in the instructions that it shouldnt be used for extended periods of time. Try searching "neckcloud cervical stretcher" or for sleeping "cervical neck pillow for sleeping" to find some better options that do not extend to the thoracic spine.

Can anyone tell me how effective this is on your rhomboids and also your upper traps? I'm really looking for something that does both.

The points on top of this product will press on your rhomboids, upper trapezius, scapular lift, and upper back erector spinae. Combined with traction on the neck. It can relax the muscle tension in the neck connected to the upper back and relieve pain. Because the muscles are connected, overall relaxation will be more effective than partial relaxation of the painful points. 
If you feel that the pressure points are too soft, you can try to sway your body from side to side, which will make the pressure points go deeper inside the muscle layers. But in the beginning I recommend a gradual approach.
