Q: Does this product let mix cold and hot water?

Yes. It has one spout so hot and cold are mixed. Pulling forward less from 0° to 45° will give you cold water to cool water. Pulling forward from 45° to 90° will give you warm to hot water.The more you pull the lever forward, the more the water temperature rises.

Q: Are the water lines connected permanently to it

it is a standard faucet with hot and cold lines that need to be attached.

Q: Did anyone else have an issue with the metal bolt sliding around on the metal washer causing the faucet to turn?

No, i haven't had that problem yet

Q: What does "forward only" handle mean?

It means all water temperatures are achieved by pulling the lever forward. The further you move the lever forward, the warmer the water gets. It also avoids the problem of the lever hitting a "too close" wall behind the sink because the full range of motion (from 0° to 90°) is achieved in a forward motion. A "too close" wall would disallow the full function of the temperature control that moves back towards the wall because the lever could potentially hit the wall before you get to the hotter temperatures.
