Q: Does it zip up or does it have button?

They have buttons. Very soft fabric. I got the beige.

Q: Does it need ironing after washing

I'm certainly not an iron queen ... I fold or put back on the bed immediately after drying. It doesn't seem to be wrinkled. The cotton is a very soft cotton unlike the muslin cotton of yesteryear.

Q: Is this duvet soft and smoothe on both sides?

It is exactly the same on both sides...100% cotton....it is a high thread count therefore very smooth...but is not to be confused with the satin cotton that is really silky and sort of shiny.
