what kind of duet cover will work with this insert's corner tabs?  How does one attach corner tabs to cover's corners?

I don't know how these work w/o sewing fabric ties on both comforter and duvet. I found the perfect answer for any duvet and it holds comforter in place w/o slipping, tearing, shifting or sewing! I found these clear hard plastic circles (doubles-one snaps inside the other ) to hold corners in place w/o any problems. I first used them 4 a med. weight comforter (not this one) and an antique lace duvet. Held beautifully and invisibly w/no damages. With the corner tabs, you just put the larger circle on the top of tab and the smaller circle on the outside bottom of the duvet cover and snap together.Voila! I can't recall the name but Bed,Bath & Beyond has a four-pack for about $10-$12.Just tell them what you are looking for and that you need clear plastic comforter/duvet cover corner holders. They also last forever and are easy to remove for cleaning or changing styles.

Should this be washed befoure using it?

Yes! Always wash your bedding before use. Your comforter could have been sitting in a warehouse for a year before it gets to you. Or it could be coming from overseas on a ship. There might be bedbugs, mites, dust & dirt on the bedding. So it's always best to wash your bedding before you place it on your bed.  
Read the tag for care instructions.
In my experience comforters hold up best when washed in cold water. On the Delicate cycle or Bedding cycle with light detergent. Fabric Softener is okay to use on certain fabrics. If you do decide to use Fabric Softener, use a little bit. Too much Softener can stain your comforter & ruin the fabric.
I always fold up the comforter before I place it in the dryer. That way the comforter doesn't snag on anything & stretch or rip.
Dry it on the low heat setting. It might take longer to dry. But too high of heat and your comforter might melt, depending on the fabric.

Does this sleep hot ?? Is it heavy ??

It is October in Maryland - 62 degrees in the bedroom. My husband and I are small, so size has nothing to do with it. This blanket sleeps hot, period. I had to throw it off multiple times, freeze, put it back on and throw it back off - all night. It isn't a heavy comforter but it doesn't breathe at all and traps an immense amount of heat.
