No, it does not come with a chainsaw, this is just the thing that you can attach the chain saw too to make boards. My husband loves it. Does a really nice job.

Q: Does this fit a 20 inch bar for Stihl chainsaw

Yes I use a 20" Stihl MS290 and it works great.

Q: What is max cut with a 18" bar

Maybe 16? The tip of the bar is free and you lose 2 inches at the motor

Q: How do you make the first cut for the saw to ride on?

I just built some wooden guide rails to make my first cut and second cut and then you can run the mill without guide and cut it at will.
