does this come with the 4 storage boxes or not ?

Yes, it came with the storage boxes. But the packaging was terrible and one entire corner was smashed and cracked. Bought something similar at a local big box store.

Is it possible to buy additional plastic dividers?

Contact Plano, tell them which box you have and they will send you a goodly number of the dividers free of charge. Great company to deal with.

The dimensions are 15.5 x 11.5 x 9.5 inches. Which is height, width and depth?

Height is 11.5, Depth is 9.5 and width is 15.5

is the top of the frame flat (not the individual cases) or does it have a lip around it?

It has a lip around it. I bought mine to replace the frame on my Mark Pak tackle bag, it had an insert to make the top flat.
