Q: Is it true that some people's bristles are hard and fall off?

I've never had hard bristles, but I have had them fall off. Usually it's only when using a new brush head and it's usually only a max of 5 bristles over several brushes (never more than 1 falling out at a time)

Q: do these replacement heads include a removal tool?

No you don’t need any tools. Just grab and pull off. Throw away and snap the new head on. Really simple. Ps, for the money these are worth buying.

Q: How does it whiten? Does it have whitening gel in it?

No gel. The brush is cup shaped to fit over teeth for a deeper cleaning over four sides. The idea that the bristles make teeth whiter is not much more than a marketing gimmick to grab attention.
