Is the handle ceramic? It looks like plastic that has just been glued on to the ceramic mug. If that's the case, it seems likely to fail.

That picture is a horrible representation of the mug and does look like it's a glued on separate piece. Mine looks nothing like that. Mine looks like it's all one piece just like any other ceramic mug.

Does mug fit in typical card holder?

NO! It's a very nice mug but does not fit in my Toyota Prius cup holder. I thought it barely fit then when I braked once it tipped over and spilled coffee everywhere.

Is this mug easy to drink out of without the lid? In others, the lip of the mug is too thick and it makes it awkward.

Yes, you can, the lip isn't very thick, however with the lid on it pushes down into the cup, so you can still drink out of the glass lip with the lid on. Best mug I ever purchased.
