Does the phone fits with a case?

Yes, I have my iPhone mini in a clear case and it fits perfectly in the holster.

can I walk with it on mywrist?

No. It’s a scabbard that fits securely on a belt. It has a strong velcro closure & is all around sturdy, but you’d have to strap it to your arm with some strap not provided by the manufacturer.

Anyone have the belt loop fail? purchased case and within 2 weeks, case and phone lost.

When you say "fail" I'm not sure if you mean broke or popped off. I've had mine for 2 years now and the belt clip has not failed. Considering it's just plastic, I am very pleased with how long it's held up. I have had it pop off my belt once or twice in the car though
