What does "nsfw" stand for?

NSFW stands for "Not Safe For Work." This expansion pack includes questions with some adult content and isn't suitable for children under 18.

I don't understand how the judge is literally holding the answer with the card and not supposed to look, are you supposed to cover the bottom half?

The Judge is absolutely encouraged to look at the answer at the bottom of each card while playing. The Judge is the only person who can see both the trivia question and answers the entire round. The Judge won’t know if any of the other players actually know the correct answer until the end of the round when answers are revealed. This means Judge must predict who answered correctly based only on each player’s explanation as to how or why they know an answer. Players are forbidden from saying any of the multiple choice answers in their explanations, but the Judge is encouraged to ask follow up questions if any explanations sound fishy. Player answers are revealed AFTER the Judge makes a “Brilliant” and “BS” prediction.

Can this be played with 7 people? If 1 person is the judge, can't there be 6 others players besides the judge?

Yes it can be played with 7 people. The game comes with 6 sets of ABC response cards. So you can make another set with pen and paper or download and print a set from brilliantorbs.com
