No, the 1928 Silver-tone Heart Eyeglass Holder Pendant Necklace, 28", is not adjustable.

Q: How does the eyeglass holder work? It's not obvious to us from the pictures nor could we find an explanation. Thanks.

Take one of the pieces of your glasses that goes over the ear. Insert it into the loop at the bottom of the eyeglass holder. Keep it, and the other ear piece folded flat against your body. Hope that makes sense. It's easier to show someone than to explain in words. :)

Q: Is this item nickel free & could it be used as a holder for push button medical alert?

Yes, it appears to be electroplated stainless steel which is a nickel compound. It could be zinc. Because it is worn outside of clothing this may not be as issue for you. It is quite heavy.If the medical alert button could be attached is hard to say. The hole is about 1 cm round. I hope that answers your question.
