sir/madam this item original packaging?

Yes, this came in a sealed Sony box as if you had purchased it from Best Buy or any other brand store. Usually if someone is selling a product that is not in its original packaging, it's usually listed on the description.

Will sound playing from a smart tv app do I need a second hdmi going to the input side of the receiver or does the hdmi out receive audio from tv

The output from the receiver will be to the TV through HDMI. It will be able to carry the audio signal. You may need additional HDMI cables for any other devices you may decide to use. The receiver is capable of switching inputs and displaying them on your TV, so the only thing you TV really needs plugged in is the receiver and maybe an antenna.
